Name Change Documentation

  • Marriage Certificate

  • Divorce Decree

  • Court Ordered Document

  • Drivers License

    • If a candidate does a score transfer online, they can upload the documentation with the order, this will be the easiest method for a candidate to do the score transfer.

ONLY EXCEPTION - Name change is not required if candidate is changing name back to name already tested under. I/e - Candidate took exam originally in 1997 as Hannah Smith and then was married and changed to Hannah Jones. If Hannah states she is now back to Smith, since this was an already established name for candidate, there is no documentation required.
