SA NSTA's ADA Request Documents Required

Original Ink Signatures:

  • Part I— does not absolutely need to be original ink, but we are still requesting it.

  • Part II—if not an original ink signature from the practitioner (ex. electronic signature, stamp), then call the number that is listed and verify that the practitioner, Dr. Office, Disability Resource Center (DRC) staff or approved professional completed the form.

    • If Part I does not list a specific practitioner but a name of an office with the address listed, and matches the address listed on Part II—we will accept as there may be multiple practitioners in one office that a candidate may be seen by.

 

First time applying for NSTA’s:

  • Need completed Part I, Part II, and a Personal Statement (PS). PS is required in this situation.

 

Repeat Test Taker:

  • If Part II is requesting the same items that were previously approved for, do not need a PS. Can process without PS.

  • If Part II of the request differs from what was approved before:

    • If requesting less than what was approved prior, request PS to verify.

    • Depending on diagnosis (DX) and if they are asking for more and does not correlate with the DX, we may require supporting doc from practitioner and updated PS. Discuss with team

 

Learning Disorder (LD) DX- first time approval required documentation:

  1. Part 1

  2. Part II

    1. By a practitioner who can vouch for the candidate’s LD DX. The practitioner can reference the completed eval as it may not be the same practitioner who completed the original evaluation.

    2. By the candidate’s DRC office

      1. If a DRC completes Part II, they must also write a letter on their letterhead, signed, that details the nonstandard testing arrangements the candidate received while attending their facility and the duration of those accommodations.

  3. Personal Statement

  4. Eval- copy of the most current psychoeducational, psychological, or neuropsychological evaluation that diagnosed or confirmed your learning disability.   

    1. If they do not have an eval, but can provide a DRC letter listing arrangements received, we will accept as it is showing a “history of”.

 

Learning Disorder- repeat candidate without eval submitted with current documentation:

  • Look up candidate in Special Exams to determine what year the candidate was approved for arrangements prior. Check previous documentation (S:\EXAM SERVICES\Special Arrangements\SCANS) to determine if we have eval in previous years.

    • If the candidate’s evaluation is in the prior approval folder, then copy/paste the eval in the current file and use.

    • If it is not, review documentation and arrangements being requested to determine if we need to ask for one:

      • If they are asking for more arrangements than what was previously approved for, need eval.

      • If they are asking for the same arrangements and state that they do not have an eval, we can request DRC letters of what they received in school to accept instead of eval. We want to “complete” the file.

 

Learning Disorder- returning candidate & still have eval on hand:

  • If the candidate email (EM) states that they want the same arrangements, we just need an updated Part I. We can send via DocuSign.

  • If the candidate submits a Part I and Part II requesting the same arrangements, we do not need a PS—process the request as the prior.

  • If we receive a Part II that is missing arrangements that were approved prior, need a PS to verify this is correct as practitioner may have forgotten to add an arrangement(s) when completing form.

    • If candidate requests an item that is not on the Part II, but previously approved for and supported in eval—we will move forward with approving per PS request.

  • If we receive a Part II that is requesting additional arrangements:

    • Check eval to see if they are supported. If not, will need a supporting letter from practitioner or updated eval requested along with a PS.

    • **Before sending EM to candidate requesting additional info—may want to discuss as team to determine next steps on what we will require

Note: We have also been considering chronic conditions (such as cerebral palsy, blind, etc.) to just complete a Part I via DocuSign if they are still requesting the same arrangements.

 

Diabetes DX- new or repeat candidate:

  • In addition to Part I, Part II and PS, need Appendix B (diabetes questionnaire in packet). If Appendix B is not complete, send them the packet to complete page 6 only.
