Score Transfer Form
If candidate calls after full year of the original score transfer request or testing – candidate will have to pay for score transfer to be processed.
If candidate has tested with ESL/ELL – NY will not accept scores- no matter which exam level taken.
If candidate accidently marked to have the master’s exam transferred to NY and mean to send to MD – CSR must take corrected information and send email to Specialist II to get sent correctly.
Older Exams Marked as Level B/C
This is submitted with any exams submitted that were taken as Level B (Masters) or Level C (Clinical)
